Ask an AI assistant which weight-loss program actually works, or whether a fertility clinic is worth the money, and the answer probably leans on a Reddit thread instead of the brand's own website. In Yolando's analysis of 27,812 AI answers across ChatGPT, Gemini, Perplexity, and Claude (June 2026), Reddit was the most-cited domain in every vertical we measured: 12% of all citations, showing up in roughly one of every three answers.

So what do you do with that? You can't buy the top spot. You can't take the thread down. And the moment you fake your way in, you're breaking federal rules. What's left is a Reddit playbook built on one idea: steer, never control.

Two quick definitions before we get into it. A "citation" is when an AI assistant names a source and links to it while building an answer. GEO (generative engine optimization) means earning visibility inside those AI answers, the way SEO earns visibility in Google's blue links. Why does Reddit win in AI search?

Reddit wins because large language models (the AI systems behind ChatGPT and its peers) treat lived experience as ground truth. When a patient asks something with no single right answer (is this treatment worth it, was recovery as bad as people say, did anyone regret it), the model reaches for the place where real people argue it out. In our dataset, that was Reddit across all eight healthcare verticals.

Scale is part of it. Reddit reported 116 million daily active users in Q3 2025, spread across tens of thousands of communities. But access matters more. In February 2024, Reddit signed a content-licensing deal with Google reported at roughly $60 million a year, giving structured, real-time access to its forums. A similar arrangement with OpenAI followed. AI engines aren't stumbling onto Reddit. They're licensed to learn from it.

Healthcare makes the pull stronger. In the 2024 Health Information National Trends Survey, 70.2% of US adults who use social media reported participating in online health communities, and more than one in five said they had made a health-related decision based on social media content. Patients want to know what happened to people like them before they book, and Reddit is where that testimony lives: public, searchable, and licensed.

Reddit is number one everywhere, but the size of its lead depends on the category. It ranged from 8.5% of citations in weight loss to 16.5% in lab testing in Yolando's June 2026 study. Now the caveat worth building your strategy around: the engines disagree. Reddit's citation share swings by platform and by week. In our data, one heavy-citing engine pulled dozens of sources per answer and leaned on Reddit, while another referenced Reddit close to 0% of the time. A Reddit-only strategy leaves you invisible on the assistants that don't favor it. Steer Reddit, but don't bet everything on it.


What does "steer, never control" mean in practice?

Steering means you influence what the community says about you by showing up honestly. Control means you manufacture it with fake accounts and paid praise. In healthcare, the second one isn't just tacky; it's illegal. The Federal Trade Commission (FTC) banned fake reviews, undisclosed insider reviews, and AI-generated testimonials in its final rule effective October 2024, and violations carry civil penalties. An employee praising your clinic in a subreddit without disclosing they work there isn't a growth hack. It's a penalty-bearing violation.

Three rules keep you on the honest side. Be transparent about who you are: if you represent the brand, say so in your profile and in the thread, because that disclosure is what makes your contribution count. Offer real expertise for free: answer the questions patients are actually asking, because usefulness earns standing and pitching burns it. And no sockpuppets, ever. One honest account beats ten fake ones, and moderators ban coordinated inauthenticity fast. Steering is slower, but it's the only version that survives an FTC inquiry, a moderator, and a skeptical patient reading the thread six months later.


How do you find the threads that feed AI answers?

Before you participate, you listen. The point is to learn which subreddits and threads AI assistants pull from when they answer questions about your category, because those are the conversations moving your visibility. Start with the questions, not the keywords. Open the assistants your patients use and ask what they would ask ("Is worth it?", "Best clinic for near me?", "What are the side effects of ?"), then note which threads surface and which communities they come from. Watch for three signals:

  1. Which subreddits recur. A handful of communities usually dominate any given healthcare category. Find yours.

  3. What the questions reveal. The cited threads are a free list of the anxieties your future patients carry: safety, cost, recovery, regret. Mine them.

Monitoring isn't a one-time audit. Reddit's citation share shifts week to week as platforms retune, so the threads feeding your answers this month may not be the ones feeding them next month. Treat it as a standing signal, not a project.


How do you earn recommendations without faking them?

The strongest position in Reddit AI search is being recommended by someone with no connection to you. When a patient volunteers "I went to this clinic, and here's my honest experience," that carries weight no ad can match. You earn those mentions the slow way: by being so useful in the communities that matter that people remember you. A clinician who spends twenty minutes answering a stranger's recovery question, clearly and patiently with no pitch, builds a reputation that gets named later in threads they never see. That's seeding done right. Plant expertise and let the community decide what to do with it.

The line between honest seeding and astroturfing is disclosure and truth. Seeding means showing up transparently and adding real value. Astroturfing means manufacturing the illusion of grassroots support you never earned: fake accounts, scripted praise, incentivized posts. The first builds durable visibility. The second gets your brand named in a "look at this astroturfing" thread, exactly the kind of citation you don't want feeding an AI answer. Useful participation compounds over time, and there's no shortcut.


What do you do when Reddit sentiment turns against you?

Sometimes the thread AI is citing is unflattering, and you can't delete it. This is where steering matters most, and you have two honest levers.

Engage directly and transparently. If a thread about your clinic contains a factual error or an outdated claim, a disclosed representative can correct the record: politely, with evidence, once. Communities respect a brand that shows up to clarify and resent one that argues to win.

Pair Reddit with a gatekeeper source. AI assistants rarely build an answer from Reddit alone. After Reddit and Wikipedia, each vertical has its own authority (a directory, a health publisher, a professional body) that engines cross-reference. When community discussion is mixed, a strong, accurate presence on those higher-authority sources shapes the overall answer.   4. Correct the record when needed. Fix factual errors in threads about you, once, with evidence and disclosure.

  5. Build authority beyond Reddit. Strengthen your reviews, service pages, and directory profiles so no single thread decides your answer.
